A newly hired student employee must come to the Student Employment Office to complete their I-9 form before the employee begins work. Departments will be charged a $100 fine and charged for each “late” I-9 reported in the university’s monthly compliance report.
The following is a list of acceptable documents that can be used to meet I-9 requirements when determining eligibility to work in the United States. All documents must be current (not expired), original and unlaminated.

For persons under the age of 18 (who cannot provide a document from one of the options listed above in LIST B), one of the following three documents may be used to verify identity: School record or report card; Clinic, doctor or hospital file; or a file for a daycare center or kindergarten.
The Social Security Administration has determined that a person can apply for a new Social Security card if his/her card is lost or stolen. Do not apply for a new card if you know where your card is located.
If you do not have an original Social Security card for IRS or I-9 purposes, you can obtain one by going to the local Social Security Administration office. The Social Security Administration is located in the J.Will Robinson Federal Office Building at 88 West 100 North, Provo UT, and is open M, T, Th, F from 9am to 4pm and W from 9am to 12pm. Once you have received your Social Security card, you will need to submit it to Student Employment in the Wilkinson Center 2024 to update your details with the University. Please note that you must surrender your card within 30 days to meet IRS requirements and up to 90 days to meet federal I-9 regulations (if you were originally hired with a temporary Social Security card receipt) .

Your supporting document must be an original or certified physical copy. A certified copy is any document that bears the seal or stamp of the official issuing authority. You must submit one of the following documents:
Paper only: You cannot submit digital proof of U.S. citizenship, such as a mobile or electronic birth certificate. You must provide physical proof of US citizenship and a photocopy of the document.
Returning your document: You must present your proof of U.S. citizenship to the acceptance agent or passport officer. We will send your document back in a separate mailing up to 8 weeks after you have received the new passport.
In addition to the original or certified copy of your U.S. citizenship, you must provide a photocopy of the document.
Disposal Of Unused Medicines: What You Should Know
If you do not wish to submit a photocopy of your citizenship certificate, you can submit a second certified copy of your citizenship certificate, which we will retain. Failure to submit a photocopy or a second certified copy of your proof of citizenship may delay the processing of your passport.
You must provide one of the following identification documents (IDs) to the underwriting agent. The ID must be able to identify you easily.
Submit a photocopy of the front (and back, if there is printed information) of any identification you present with your application.
